Although by no means comprehensive, this list will assist in identifying some of the more well-known surveys,  scales and other measures developed at Duke University Medical Center. Click the information icon for even more information if available. 

  • Abbreviated Duke Social Support Index  
    A shortened 11-item version of the Duke Social Support Index (DSSI)
  • Computerized Duke Severity of Illness Checklist.  
    A computerized version of the Duke Severity of Illness Checklist designed to assess respondent's burden of illness at a given 1-week point of time.
  • Duke Activity Status Index (DASI)  
    A self-administered questionnaire that measures a patient's functional capacity.
  • Duke Age Awareness Item  
    Reflects subjective feelings of age awareness, which is another way of stating that individuals are as old as they feel.
  • Duke Anxiety-Depression Scale (DUKE-AD)  
    A brief survey used to screen for anxiety and depression in the primary care setting. This survey is a subset of the larger Duke-UNC Health Profile (DUHP).
  • Duke Case Mix System (DUMIX)  
    Combines age, gender, patient-reported perceived and physical health status, and provider-reported or auditor-reported severity of illness to classify patients by their risk of high future utilization.
  • Duke Depression Evaluation Schedule (DDES)  
    A psychosocial interview and life events checklist; also called Duke Depression Evaluation Schedule for the Elderly (DDESE)
  • Duke Health Profile (DUKE)  
    Intended as a brief and practical measure of patient-reported health and dysfunction in a primary care setting.
  • Duke Health Profile-Adolescent version (DHP-A)  
    Used for the assessment of health status in adolescents.
  • Duke Semantic Differential Technique  
    Measures self-concept and self-esteem.
  • Duke Severity of Illness Analog Scale (DUSOI-A)  
    The DUSOI's scale for measurement of severity and comorbidity.
  • Duke Social Support and Stress Scale (DUSOCS)  
    Rates family and nonfamily relationships in terms of the amount of support they provide and the amount of stress they cause.
  • Duke Social Support Index (DSSI)  
    Used to measure social support of the elderly.
  • Duke Social Support Index--"shortened"  
    An abbreviated 11-item version of the DSSI.
  • Duke University Index of Religiosity (DUREL)  
    Measures religious involvement and intrinsic religiosity; also called Duke University Religion Index (DUREL).
  • Duke University Severity of Illness Checklist (DUSOI)  
    An instrument for measuring illness severity and comorbidity in ambulatory patients.
  • Duke-UNC Functional Social Support Questionnaire (DUFFS)  
    Measures a person's satisfaction with the functional and affective aspects of social support.
  • Duke-UNC Health Mini-Profile (Mini-DUHP)  
    Derived from the 63-item Duke-UNC Health Profile (DUHP) and is designed to measure functional health status in adults aged 18 years or older.
  • Duke-UNC Health Profile (DUHP)  
    The 63-item parent document of the Duke Health Profile (DUKE). It measures adult health status in the primary care setting on four dimensions: symptom status, physical, emotional, and social function.
  • Duke-University of North Carolina (UNC) Functional Social Support Questionnaire (FSS)  
    The DUFFS; measures a person's satisfaction with the functional aspects of social support.
  • Duke-University of North Carolina Health Profile--Functional Status Subscale.  
    A subscale of the Duke Health Profile (DUKE).
  • OARS Multidimensional Functional Assessment Questionnaire  
    Assesses the overall personal functional status and service use of adults, especially in the elderly; also referered to as OMFAQ.
  • OARS Multidimensional Functional Assessment Questionnaire--Instrumental Activities of Daily Living  
    A part of the original 1975 measure.
  • OARS Multidimensional Functional Assessment Questionnaire--Modified Version  
    A revised version of the Center for Aging and Human Development's (1975) measure.
  • Older Americans Resources and Services--"selected items"  
    Comprised of "3 self-perceived health questions from the 1975 survey.
